* test/runner.rb: reporting test coverage for test-all with COVERAGE env.
[Feature #10189][ruby-core:64681]
* Makefile.in: added task for coverage report.
* common.mk: added definition of forked simplecov url.
* .gitignore: ignored coverage directory.
