move fields from `th` to `ec`.

* vm_core.h: move rb_thread_t::passed_block_handler to
  rb_execution_context_t::passed_block_handler.
  Also move rb_thread_t::passed_bmethod_me to
  rb_execution_context_t::passed_bmethod_me.
